In order to participate on any teams in the Parkside Athletics Program, all students trying out for a fall, winter or spring sport MUST:

1. Have a completed WCBOE Student Athlete Physical packet and on file with Cindy Riley, the Athletic Trainer for Parkside High School.

2. Physical documents can be scanned in and emailed to Cindy Riley.

Most of the information will be completed by the student or parents, but the physical must be completed by a doctor. Once it's turned in for a school year, it won't need to be repeated for a later sports season.
